Before the Governor murders all of the other soldiers on Welles’ team, the helicopter pilot is alive and well with Dr. Stephens the last time we see him. Being told that all of his friends will be saved if he reveals where they are, Welles responds accordingly. He stays behind to rest while the Governor goes to meet with the other guardsmen.

Obviously, the Governor only winds up killing all of them instead. And at the end of the episode, we learn that Welles didn’t fare any better. When the Governor takes a seat in front of his collection of zombie heads in fish tanks, we can see that the decapitated head of Welles is among them. Whether it was before or after he killed the others, the Governor had Welles decapitated and kept the head as a keepsake.
