For years prior to the zombie apocalypse, Otis was working as a ranch hand at Hershel Greene’s farm. He also has experience as an Emergency Medical Technician, sometimes working with the fire department. We also know he is an avid hunter and apparently a great guitar player as well.

After the outbreak, Otis and his wife Patricia stay with Hershel’s family at the farm. While hunting a deer in the nearby woods, Otis accidentally shoots Carl. Visibly distraught, Otis has Rick carry Carl back to Hershel’s farm. Using Hershel’s medical knowledge, Carl is saved for the time being. However, he needs more medicine, which the Greene family doesn’t currently have.

Otis feels terrible about shooting Carl, but knows where he can find more medicine: a FEMA outpost nearby. He travels there with Shane, refusing to let Shane go alone. When they arrive, they discover that the building has been overrun with walkers. But leaving without the supplies is not an option for either Otis or Shane. They manage to evade the walkers and get the meds, but getting away turns out to be an issue.

Shane and Otis grow too fatigued to successfully escape an incoming pack of walkers. Shane tells Otis to take the supplies and move on, leaving him to die. Otis, however, refuses to leave Shane behind. Soon after, the duo realize that they each only have one bullet left. This is when Shane faces the moral dilemma of dying with Otis (thus killing Carl too), or sacrificing Otis to get the meds to Carl. Shane chooses the latter, shooting Otis and leaving him to be devoured by walkers.
