After being separated from the group at the end of Season 2, Andrea was never really able to find her way back home to them. She was too involved in Woodbury to simply walk away, not realizing at first how far gone the Governor was. When she made the choice to go back, it was too late. He captured her and left her to die by trapping her in a room with a zombified Milton.
By the time Rick and Michonne find Andrea, she’s already been bitten by Milton. There is nothing they can do but offer her a gun for her to take herself out with. She takes Rick’s pistol, noting that she knows how to turn the safety off. It’s a throwback to the first time they met, making this death sting just a little more.
With Michonne by her side, Andrea takes the pistol and shoots herself with it to prevent herself from becoming a zombie. As she said in her dying moments, she tried her hardest to bring peace between the two groups. But it wasn’t meant to be and it even got her killed.
